How to Report a Lost Card
You can report a lost card by
phone or in person. Until a card is reported lost, the cardholder is responsible for all materials checked out to the card, including any items that have been claimed to have been returned. A valid form of identification is required before a replacement card can be issued.
Please note the following:
- A $2.00 service charge shall be assessed for all replacement cards. The fee is due at the time the replacement card is issued.
- All overdue charges must be paid down to at least the overdue charge limits, plus the $2.00 service charge, before a replacement card is issued.
