Acceptable forms of identification include:
Non-Maricopa County residents must purchase a fee card for $40.00.
An individual (adult or minor) must be present to receive a library card. A parent's or legal guardian's identification can be used to obtain a card for children 11 years and younger. Any person age 12 to 17 who does not have identification or responsible party present may have a temporary card issued for the day with limited loan privileges; a permanent card will be mailed to their home address.
How to Report a Lost Card
You can report a lost card by phone or in person. Until a card is reported lost, the cardholder is responsible for all materials checked out to the card, including any items that have been claimed to have been returned. A valid form of identification is required before a replacement card can be issued.
Customers may replace a library card free of charge.
Business Cards
Business cards are available for free for businesses in the city of Phoenix. Businesses in Arizona, but outside of the city of Phoenix, are charged $40 for each card issued. Businesses are limited to a maximum of 5 cards.
School Cards
School Cards are offered to teachers to benefit students in classes which regularly visit the Library. Cards can only be used to check out books selected by students during the class visit. Each principal / director of a school must apply and sign a letter of agreement before school cards can be issued.
